Also called a “talent release” or “photo release” form. One interesting way it’s been done is to put a checkbox on the tournament registration form: “All official photographs taken of the tournament and participants are property of the organizers. Please check this box if you DO NOT consent to your child’s image appearing on official tournament-related websites and material to promote the game of chess” (or some such verbiage).
Here is a MS Word document that is a good sample of what you’ll need. The schools in my area have a policy of only putting first names on web pictures if any names at all, and that may be a good caveat to add to your form. Like I said though…it’s so much easier to add a waver to the tournament registration form. Good luck!
